Dear
Players and Parents!
For 2006-2007 JO season we will again use on-line
registration.
In order to be registered, you have to click on the link above and
follow the instructions. Please follow the appropriate link depending
if you were or were not registered last year. Once you fill in required
information on the first page you will immediately receive an e-mail
with a new ID and password. Use them to enter registration pages.
On the second page you will need to choose the club. If you are
trying out only for Memphis Metro, please choose Memphis Metro JVC,
Inc. If you are going to try out for several clubs, please choose
"UNDECIDED". Further, enter Regular Membership type ($35),
parents / guardian names and choose one preference in addition to
playing. Unless you really want to pay any additional fees do not
click on HP and referees fee. On the next page check if all information
is correct, edit if necessary, click I agree box and click confirm.
Print the Membership Application Confirmation and click on the Waiver
& Code of Conduct Form at the left bottom corner. Print and
read those 2 pages, sign (PARENT AS WELL AS PLAYER). GO BACK and
click on and print 2007 Player Medical Release Form. Fill in all
required information, sign and send together with check to Memphis
Metro JVC, Inc., POB 2287, Cordova, TN 38088 by October 20, 2006.
Tryout
fees:
$70 if registration mailed by Oct 20, 2006
$80 at the door
Registration
fee includes:
$35 USAV non-refundable fee
$35 Memphis Metro non-refundable tryout fee ($45 at the door)
Summary:
After successful registration you are going to have 3 printed forms:
1., Membership Application Confirmation (1 page)
2., USAV Participant Code of Conduct (2 pages)
3., 2007 Player Medical History and Release Form (2 pages)
4., $70 check to Memphis Metro (we will send $35 to Delta region
office)
IF
YOU TRYOUT AT DIFFERENT CLUB BEFORE OUR TRYOUTS PLEASE BRING RECEIPT
YOU PAID $35 USAV FEE. OTHERWISE WE WILL HAVE TO COLLECT THAT FEE
AGAIN.
